What is TENS Electrostimulation?

TENS electrostimulation is a clinically proven, non-invasive and non-pharmacological analgesic therapy. It is widely used in relieving both chronic and acute pain. TENS treatments are performed today practically in most physiotherapy rooms, but the TENS method is particularly popular thanks to the availability of small and portable TENS devices that can be used individually by the patient at home.

What is pain?

Pain is part of a natural warning system against adverse effects, such as further injury. It is therefore necessary in the process of defense against harmful factors. However, long-term persistent pain (often called chronic pain), previously diagnosed, no longer fulfills its function, becoming the cause of suffering. It can be eliminated by surgical, pharmacological or electrostimulation methods to continue enjoying life.

How is chronic pain treated?

As already mentioned, the treatment of chronic pain can be carried out by various methods. Since pain is always felt subjectively by every organism, there is no universal method of treatment. Many of these treatments have harmful side effects. Medications over a longer period of use can cause addiction, and surgical intervention always carries the risk of complications. TENS stimulation offers a unique method, alternative to other methods of pain relief and most importantly, it is closest to natural.

What is TENS?

TENS (Transcutaneous Electrical Nerve Stimulation), or transdermal nerve stimulation, is a method used by electrostimulators that send low-amplitude electrical impulses from electrodes through the skin to peripheral nerves. These are the nerves responsible for feeling the temperature. Through these nerves, the electrical signal reaches the synaptic connections of the spinal cord, where it acts in two ways:

  1. As a signal blocking the nerve impulse carrying information about pain to the brain, so that the brain does not receive data on the place and severity of pain. This effect is triggered by high frequency TENS pulses. Blocking pain impulses does not damage the structure of the nerves, but "cheats" the mechanism of informing about pain. The mechanism of blocking nerve impulses carrying the signal of pain was adopted by science in the 60s of the twentieth century as the theory of pain gates Melzack and Wall. Type C nerve fibers carry the signal from the electrostimulator faster than type A fibers, so that the signal from the stimulator wins in competition for access to the hypothalamus.
  2. As an initiator of the formation of natural analgesic substances – beta-endorphins, having analgesic effects as strong as morphine (or other opiate compounds). This mechanism triggers low-frequency pulses or short strokes. burst).

As we can see, TENS does not cure the causes of pain, but significantly reduces its sensation and the analgesic effect persists for a long time.

  • significant reductions in the amount of drug intake (from 40 to 80%) and related side effects

Types of TENS pulses:

  • low frequency /1-2 Hz/
  • high frequency /50-200Hz/
  • with short pulses /0,01-3ms/
  • with long pulses /10-60ms/

TENS low frequency (APL-TENS / Acupuncture Like TENS / AL-TENS), are electrical impulses used in electrostimulation, which have a relatively low frequency (lower than 10 Hz, and usually 2 - 4 Hz) and a fairly high intensity - up to 100 mA (sometimes it borders on pain). Pulse duration - 200 microseconds. The procedure takes about 25 minutes. The pulse amplitude is modulated. Current is used with such values as not to cause contractions mięśni.Skurcze are undesirable and indicate poor selection of impulse parameters. Small electrodes are used in places typical of acupuncture.

TENS high frequency, or conventional TENS (C-TENS), is characterized by a relatively high pulse frequency - between 10 and 200 Hz and a low intensity - from 25 to 30 mA. The duration of the pulse is 50 to 100 microseconds and the duration of the procedure is quite varied - from 30 to 90 minutes. Electrodes are placed over the pain area, and if this is not difficult or impossible, they are applied proximally (closer to the spinal cord) relative to the location of the pain, directly above the nerve supplying the painful area. In the first period, the patient experiences a slight tingling, followed by analgesic action (there should be no contractions of the surrounding muscles).

impact TENS (BURST) – that is, strikes with series of pulses. It is a modern method used in the latest cameras, usually consisting in the application of two strokes of several pulses. The most commonly used pulse frequency is 200 Hz. It is more tolerated by patients, and the analgesic effect lasts up to 4 hours. It is a type of impulse used especially in the treatment of deep pain and chronic pain. Due to the delayed occurrence of the analgesic effect, it is recommended to use originally classic TENS, followed by shock pulses.

TENS short intense (Brief Intense or HI-FI TENS). This is quite similar to high frequency TENS. The pulse frequency of 100 Hz is used here, and their duration is 200 microseconds. Attention, the intensity here is the highest of the human tolerated and causes tetanus contraction and anesthesia after about 15 minutes.

TENS modulated is a method when one of the parameters (frequency or intensity) is modulated. This method of modulation gives the impression of massage. The duration of the procedure is usually 15 minutes. This method is most often indicated in hyperaesthesia. It can be used when traditional or impact TENS does not bring positive effects. The change in frequency effectively prevents accommodation, and the analgesic effect is delayed and less intense compared to other types, but the effect achieved is quite stable. It is recommended to use it after traditional TENS.

Use of TENS

Is this method safe?

YES . Although the thought of electrical impulses sent to the body may seem seemingly frightening, in fact the patient feels only a slight tingling on the skin and at most a slight vibration of the muscles around the electrodes. TENS pulses have low intensity and stimulate only sensory nerves on the surface of the skin.

Unlike many drugs, TENS currents have no known side effects, which is an important advantage of TENS from the point of view of people who react poorly to pharmacotherapy.

In very exceptional cases, some people may experience slight allergic reactions to the conductive gel of the electrodes.

Restrictions on use, i.e. who cannot use TENS?

Despite the high safety in the application of the method, there are some limitations. People with an implanted pacemaker or patients in the first two trimesters of pregnancy should not use TENS devices (stimulation of the abdominal area is contraindicated throughout pregnancy, and the so-called. Birth TENS is used only on the back and only in the perinatal period). Distributed visceral pain or psychogenic pain are also a contraindication to the use of this therapy.

TENS should be used with caution and only for medical indications in:

  • damage to the heart muscle and arrhythmia (avoid stimulation through the chest)
  • mental illness and emotional states
  • epilepsy (avoid stimulation through the scalp and neck)
  • avoid stimulation of the cervical sinus area (may cause hypotension - lowering blood pressure)

Caution should be exercised when applying electrodes:

  • on the eye area
  • mucosal surfaces
  • skin cavities
  • around the neck - throat and larynx
  • in children
  • do not perform treatments near the impact of microwaves and short waves - they can cause interference to the device. Ask your doctor if this type of electrotherapy is right for you.

Is TENS right for me?

It depends on the specifics of the problems that affect the body. The following are typical indications for the use of TENS electrostimulation:

  • Pain in the cervical spine
  • Postoperative pain
  • Pain from amputations
  • Chronic pain in muscles and joints
  • Phantom pain
  • Tendon inflammation
  • Shingles
  • Pain in the torticollis
  • Pain after fractures

NOTE: The body's response to TENS currents can vary from person to person due to different sensitivity to electrical impulses. This means that the effectiveness of TENS in some people may be lower. Of course, it also depends on other factors, such as the correct selection of pulse parameters or the way the electrodes are arranged.

In people less sensitive to TENS currents in a given area of application, it may turn out that inny type of electrostimulation, which are interference currents, which create stimulating stimuli in the deeper layers of tissues (so-called. deep TENS, although the use of interference currents goes beyond typical analgesic effects). For more advanced analgesic therapies, it is recommended to use mixed methods, e.g. TENS, interference currents, or microcurrents. In typical situations, however, the TENS method has the best analgesic effect.

Arrangement of electrodes

Electrodes can be arranged:

  • local (in the area of pain)
  • paraspinal
  • specific points (acupuncture or motor skills of muscles and nerves)

To perform the TENS procedure, you need at least 2 electrodes (one channel), which form a closed electrical circuit. Most TENS devices have two channels (4 electrodes in total), which allows you to stimulate two different areas or one area, but in a way that gives you more confidence in proper stimulation.

On the market you can also find simple "long-form" electrostimulators with an point electrode (one channel). Although these solutions are very convenient, their effectiveness is much lower in most applications.

The effectiveness of TENS depends not only on the proper characteristics of electrical impulses as a stimulus affecting sensory nerves, but also on periodic differentiation of the stimulus by changing specific impulse parameters. The basic parameters of TENS currents include:

  • frequency: up to 200Hz
  • pulse width: 50-250 microseconds
  • amplitude
  • modulation - simultaneous change of all the above parameters, including the so-called. impact (burst).

Are TENS devices easy to use?

YES . TENS devices are the size of a mobile phone and are almost as easy to use. Your doctor will usually inform you about the use of the TENS device and how the electrodes are used and how they are arranged. Also in the instruction manual of TENS devices there are usually treatment protocols for selected pain. Some programs can be modified or set individually according to your needs.

In some cases, it may be difficult to install electrodes, e.g. around the cross, especially when any inclinations and twists of the torso increase pain. In such situations, it is worth using a special lumbar belt for electrostimulation, which will not only facilitate the placement of electrodes in the right place, but also stiffen the spine.

How long can I use the TENS device?

They can be used for years. However, it should be remembered that after some time the body accustoms sensory nerves to a stimulating stimulus (habituation), which weakens the effectiveness of long-term therapy. Therefore, it is important to constantly differentiate the stimulus by changing the stimulation parameters. This can be done by selecting another program available on the device or by manually changing the selected parameters.
